A self-declaration form has been published on the Region’s website, to be sent to your doctor (for residents), or to the Prevention Department of the Local Health Authority of the province in which you are staying (for tourists).

It is necessary to indicate the dates of arrival and departure, the places from which you come and the municipalities in which you will be staying, as well as if you are in the company of your family or other people, the vehicle used and if during the holiday you will make further trips to other regional locations.

The PDF must be kept for the entire duration of the holiday; at the same time, you must keep with you, for another thirty days, a list of the places visited and the people you met during the holiday
